Under-eye bags
Under-eye bags are not one condition but a shared appearance with different causes. The most common is orbital fat moving forward with age, but fluid, a tear trough hollow and bags on the cheekbone can look similar. Treatment is chosen by cause.
Causes
- Fat prolapse: orbital fat moves forward as its supporting membrane weakens. The most common cause of bags that change little during the day and become more visible with age. It can appear young in people with a family tendency.
- Fluid: swelling that is worse in the morning and improves during the day, increased by salty food, poor sleep, allergy and fluid retention.
- Tear trough hollow: a dip at the junction of lid and cheek can make the tissue above it look like a bag.
- Malar (cheek) bags: swellings on the cheekbone, below the lower lid. They usually do not improve with lower blepharoplasty.
- Thyroid eye disease: can enlarge the fat and muscle around the eye; it is treated differently.
Telling them apart
- A bag that becomes more prominent on looking up is usually fat.
- Swelling that clearly improves during the day suggests fluid.
- A shadowed line with no bag is usually a hollow.
- Swelling over the cheekbone, below the lid, is a malar bag.

Treatment options
- Lifestyle and observation: for fluid-related swelling, sleep, less salt and allergy treatment come first.
- Creams: creams do not remove fat bags, but some can reduce how visible they look by firming the skin and reducing fluid.
- Filler: where there is a hollow without a bag, filler can soften the junction in suitable patients. It does not remove a true fat bag and can sometimes make it more visible.

When is surgery considered?
When the bag does not change during the day, is due to fat and clearly bothers the patient. A sudden, painful or one-sided swelling under the eye is a medical finding to be examined, not a cosmetic concern.
